I understand that $1000 one-time implementation fee will be billed and/or charged at the time the form is received. This one-time implementation fee includes the DMS team setting up my Organizations Audit Management Workflow for one (1) company that will be used during the Organization training. A member of the DMS team will contact my organization to obtain the information needed for set up.

Additional Audits Pricing*

This subscription level includes 50 audits per month. An additional fee of $1.00/Audit applies to audits in excess of 50 per month and will be billed on a separate invoice.

Data Validation Platform Features

Analyze and audit data points between two (2) files and or worksheets within a file. (Supported File types are .xlsx, .xls and .csv)

Service Agreement

Payment Terms: This service is a monthly subscription-based model with an annual auto renewal. The credit card will be charged on the 5th day of each month. Should the 5th of the month not fall on a business day, payment will be executed on the business day immediately prior to the 5th. If opting for the annual payment option, your card will be charged the 5th day of the month following receipt of the subscription form for the annual payment amount. The renewal will occur 1 year following the subscription date. This service has a 1-year rate lock from the date of subscription. If the subscription level changes the applicable upgrade fees will be assessed on the transaction the 5th of the month following the upgrade.

One-Time Implementation Fee: I understand that $1000 one-time implementation fee will be billed and/or charged at the time the form is received. Assurance: By signing the below, I as the customer, certify I am an authorized user of the credit card provided and as such, there will be no disputed for these scheduled transactions with the bank or credit card company; so long as the transactions correspond to the terms indicated in this subscription form. In addition, as the customer, I will provide Data Management Solutions with updated payment information promptly. Failure to comply may result in a service interruption.

Professional Liability Insurance: Data Management Solutions LLC shall obtain and always maintain during the prosecution of the work under this Agreement Professional Liability insurance to include Errors and Omissions and Cybersecurity.

Termination: Data Management Solutions or Customer may terminate this Agreement by providing thirty (30) days written notice (“Termination Notice”) to the other party. Cancellation requests should be submitted via this Termination Form. Billing will cease the billing cycle following or coinciding with the 30-day period. Example, if the termination is received on the 10th of February, billing will be complete on March 5th and cease with the April billing. If the termination is received on February 1st, the final billing will be February 5th.
